What Is PRP Therapy for Foot and Ankle Pain?
PRP injections use your own blood to accelerate healing. How it works, what conditions it treats, and what the evidence actually says.
Maybe you have a tendon or ligament injury in your foot or ankle that just will not heal, despite rest, physical therapy, and perhaps a cortisone injection or two. If so, your doctor may have mentioned PRP therapy. It has become one of the most discussed treatments in sports medicine, and one of the most confusing. Here is what it actually is, and an honest look at whether it works.
What Is PRP?
Platelet-rich plasma (PRP) is a concentrate made from your own blood. A small sample is drawn, much like a lab draw, and spun in a centrifuge (a machine that spins the blood at high speed to separate its parts). The result contains a high concentration of platelets, the blood cells that start the healing process, along with the growth factors that drive tissue repair.
That concentrate is then injected directly into the injured tissue, guided by ultrasound so the placement is precise. The goal is simple to state: deliver a concentrated dose of your body's own healing signals to the exact spot that needs them.
What Conditions Is It Used For?
PRP is mainly used for chronic soft-tissue injuries, the ones where the body's own healing has stalled. In the foot and ankle, the common uses are chronic plantar fasciitis (an inflamed tissue band under the heel) that has not responded to standard care. Achilles tendinopathy (a chronically worn, painful Achilles tendon). Long-standing ankle ligament injuries and instability. And other worn tendons, such as the peroneal tendons on the outer ankle and the posterior tibial tendon that supports the arch. PRP tends to fit tendons and ligaments best. Those tissues have naturally poor blood supply and heal slowly on their own. It is less suited to bone problems, joint arthritis, or fresh injuries.
What the Procedure Looks Like
The whole visit is done in the office. A small amount of blood is drawn from your arm and processed in the centrifuge. Then the concentrate is injected into the target area. The injection itself is brief, similar to a cortisone shot. Most patients describe pressure with mild discomfort.
Unlike cortisone, PRP works by stirring up a healing response rather than shutting inflammation down. That means the area may feel more sore for a few days after the procedure. That soreness is expected. It is the treatment doing what it was designed to do.
How It Compares to Cortisone
Cortisone is an anti-inflammatory. It calms swelling and pain quickly, often within days. But it does not heal the tissue. Repeated cortisone injections can actually weaken tendons and ligaments over time, which is why most physicians limit how many they will give in one spot.
PRP takes the opposite approach. Improvement usually builds over weeks rather than days, because the treatment is stimulating actual tissue repair instead of masking symptoms. When it works, the results tend to last longer, because the tissue itself has been strengthened rather than quieted.
For an acute flare, cortisone may be the better tool. For a chronic problem where the goal is durable healing, PRP is often the more logical choice.
What the Evidence Says, Honestly
The research on PRP is genuinely mixed, and we would rather tell you that than oversell it. The strongest signal is in chronic plantar fasciitis. A 2020 meta-analysis in the American Journal of Sports Medicine (Huang and colleagues) pooled twenty trials of PRP versus cortisone. For plantar fasciitis it found a clinically meaningful advantage for PRP in long-term function, though the authors rated the quality of the underlying evidence as low. In the short term, the two performed about the same.
The Achilles story is more sobering. A 2010 double-blind trial in JAMA (de Vos and colleagues) compared PRP with a saline placebo in chronic Achilles tendinopathy. Both groups did a proper exercise program. PRP was no better than the placebo. Later work has kept that debate open. So for the Achilles, we present PRP as an option some patients still choose after standard care fails, not as a proven fix.
Results also vary with the preparation method, how severe and how old the injury is, and the patient. It is not a guaranteed fix, and not everyone responds. For the right candidate with the right condition, it can be a meaningful step when other treatments have plateaued. That is the whole claim, and we will not make a bigger one.
What to Expect After
The injection site may be sore for several days. Anti-inflammatory medicines such as ibuprofen and naproxen work against the healing response PRP is trying to trigger. So we ask you to avoid them at first, and we tell you at the visit when they are fine again. Acetaminophen is fine for pain in the meantime. Most patients return to normal daily activities within a few days, with a gradual return to exercise over the following weeks.
Some patients feel real improvement after a single injection. Others do better with a short series spaced weeks apart, depending on the severity of the condition. We set that plan together and reassess as we go.
The Bottom Line
PRP therapy uses your body's own biology to encourage healing in tissues that have been slow to recover. It is not magic, and it is not right for every condition.
